The forward step down test evaluates for patellofemoral pain syndrome. The patient stands on a roughly 20 cm box using the affected leg, hands on hips, and performs a series of slow, controlled single-leg squats at a pace of one squat every two seconds for 5–10 repetitions. The test is considered positive if the maneuver reproduces the patient’s characteristic pain.
